Appointment Process

Making an appointment is an easy three step process:

1.  Read the Confidentiality Agreement .
 
2.  Complete the Consultation Questionnaire and check box that you have read the Confidentiality Agreement.
 
3.  Schedule Appointment / Make Payment .
